This study has seven objectives aimed at addressing the problems and issues observed in the Rural Health Unit of Pandacaqui. Among the issues identified are vaccine hesitancy and lack of awareness regarding the benefits of vaccines for individuals, children, and adults. The first objective focuses on enhancing the functionality of the systenm by implementing a reservation system for different vaccine types, including those for pregnant women, senior citizens, infants, and females. The vaccines included in the system are anti-flu and pneumonia, BCG, MMR, MR, Pentavalent, Tetanus, and Diphtheria. The next objective involves developing a system that can accommodate the needs of patients who desire to receive vaccines but face scheduling conflicts or other obstacles preventing them from doing so. Subsequently, the study aims to improve information dissemination by providing notifications and confirmation of reservations, including the selected date and time based on available slots. Finally, the system will be tested and evaluated by IT experts, RHU staff/administrators, and the patients or users, as outlined in the last two objectives. A quantitative method was employed in this study, utilizing survey questionnaires to assess the performance of the system. Purposive sampling was employed to select a specific number of respondents. Various research instruments, such as survey questionnaires and internet research, were utilized to gather information, enabling the successful completion of the study. The Likert scale employed in the study consisted of four verbal interpretations: strongly agree, agree, disagree, and strongly disagree. Based on the evaluation results from IT experts which the general mean is 3.56, while 3.73 are the overall mean for RHU Admin/Staff, and 3.12 are the total mean for the patients, all the objectives were successfully achieved. The system is fully functional, and the descriptive ratings provided by the three end-users indicate strong agreement regarding the system's helpfulness, user-friendliness, and reliability in scheduling vaccinations and disseminating information. The implications of the system suggest that the notification system via SMS is operational, as well as the dissemination of information through email and SMS.
