"STUDENTS PERMANENT RECORD MANAGEMENT SYSTEM FOR PANDACAQUI ELEMENTARY SCHOOL", Adviser: Dr. Johnny L. Miranda Pandacaqui Elementary School (PES) is one of those educational institutions in Pampanga which still use manual procedures when it comes in restoring important student's data, as well as transmitting documents. PES which has been using manual and traditional processes faces various issues in terms of keeping these documents safe and undamaged from uncertain yet inevitable situations such as heavy rains or typhoons that may cause flooding and fire, misplacing of documents, lack of room or suitable office in storing mass paper documents, and some problems which may result to damage or deterioration of paper. The primary aim of this project is to develop a system that can manage the permanent records of the students of PES through online transaction. Moreover, it is expected to improve each procedure and turn the manual and traditional processes into more modem method, as it faces various issues with regards to keeping these documents safe and undamaged from uncertain yet inevitable situations. The methodology used in this project was descriptive design, while programming languages used to develop the system was Hypertext Markup Language (HTML), Hypertext Processor (PHP), Cascading Style Sheet (CSS) and My Structural Query Language (MySQL). Th process of gathering data was through conducting interviews, observing and researching online, which all served as instruments to come up with a solution to solve the problems, identify the design and to suggest improvements needed to apply. Based on the tabulated results of the surveys, most of the respondents evaluated the developed system designs, functionality and security as "Strongly Agree" that garnered a mean of 3.90 for parents or students and 3.78 for teachers which indicate that the system met the required functionality in producing and managing the permanent record of students. Therefore, the developed system has a significant benefit to the school when it comes to keeping, retrieving, issuing, and transferring of documents.
