Bravo, MECpE The goal of the study, "Online Contact Tracing Application for Pampanga State Agricultural University through the Implementation of QR Code Scanning," was to create and develop an application that would assist Pampanga State Agricultural University in reducing manual recording and making it easier to locate patient records by using the application. It was faster and easier if the records of the students were shown through the application rather than manually inquiring. A contact tracing web application is an application that records and stores the user's activities. It was intended to provide the administrator with assistance through the recorder logs and records. The application was created with PHP, JavaScript, and JQuery as webbased scripting languages, XAMPP and MySQL as the database, HTML, CSS, and Bootstrap for system beautification. To analyze the problem and determine the flow and output of the program, the researchers used Data Flow Diagrams (DFD), Context Diagrams, Use Case Diagrams and System Flow Charts. The researchers also used various techniques and instruments to gather information, such as interviews and internet research. The study's locale was the Pampanga State Agricultural University. The researchers collected data through survey questionnaires. Based on the results, the system for Contact Tracing was determined to be highly beneficial to the University. According to the feedback on the application, "100% of the student respondents agreed that the system was helpful in terms of tracing students who had the symptoms, 100% agreed that the web application helped the university with contact tracing; and according to the feedback of IT students, the system got 98.57% of yes positive feedback from the respondents and 1.47% answered no. While the nurses and doctors got 96.67% of yes positive feedback from the system and 3.33% answered no. All parts of the evaluation form of IT experts that consist of an acceptable, understandable, perceivable, and operable system got 100% complied. The IT expert and staff who evaluated and assessed the system's performance aided the researchers in completing the study. The application was a great aid for the staff of the Infirmary of Pampanga State Agricultural University in easily recording patient information and reducing the need for manual recording.
