This study was to design a web-based system that will automate the customer feedback processes. The system is capable of computing the results of the customer feedback form and will generate needed reports. Also, evaluate the functionality of the system. In using a descriptive method in their observation and gathering information about the manual process of the customer feedback, were continue the interview and research, all these information were used to develop a webbased customer feedback system. The design uses a different diagram to illustrate the flow of the data and the overall functionality of the system. In creating a system the process was done by coding the integration of the system. Brackets Software was used for coding, PHP was used as a programming language platform, HTML, XML, CSS, JavaScript, Ajax, and Bootstrap were utilized. My SQL was used as a database for the system. Based on the information gathered and upon analysing successfully met the expectations and objectives. The customer feedback system for PSAU will lessen the problem that they might encounter in gathering feedback from customer.
