Formatted contents note This study was about the PSAU Veterinary Clinic's pet information management system, which used a Quick Response Code for identification. This project was a documentation component of the developed system for managing pet information as well as its owners. The PSAU veterinary clinic would benefit from the system. It would give the pet an identification card with a quick response code as a unique identity. This research intended to speed up the visitation process by transitioning from a paper-based and manual process to a technological and automated one. The quantitative research method was used by the researchers. interviews, internet research, and surveys were utilized to collect data. Analytical tools such as the System Flow Diagram, Data Flow Diagram, and Entity Relationship Diagram were also used to design the system. The researchers developed a system with HTMLS5, CSS3, Javascript, PHP, and other applications and technologies. According to the findings of this study, the total mean of the evaluations computed by the statistician for PSAU Vetmed doctors was 4.6, interpreted as Excellent. Therefore, the system was determined to be useful in managing pet information.
