Formatted contents note The study titled "Patient Record Management System with Telemedicine" was presented to the College of Engineering and Computer Studies. The system was designed to create a patient record management system with telemedione feature for infirmary of Pampanga State Agricultural University medical and dental departments. The researchers used different techniques such as interviews and intemet research to gather all the information needed for the development of the study. For the design and development of the system, different softwares and lanquages such as JavaScript, MySQL, Notepad++, Adobe CS6, Domain Name System (DNS), Ptatform, and bandwidth. The System Development Life Cycle (SDLC) using the waterfall software development process model was used. To determine the design and flow of the system, different diagrams were also used such as Use Case Diagram, Data Flow Diagram, Entity Relationship Diagram, and System Flow Chart. Two sets of questionnaires were used for the evaluation of the designed system, first is for the PSAU students and staffs, and the second is for IT experts. Based in the evaluation a grand mean of 3.78 was obtained from the users and a grand mean of 3.55 was obtained from the IT experts, which shows that both the users and the IT Experts strongly agreed that the system is useful, reliable and met all the expected functionalities.
