"WEB-BASED CUSTOMER FEEDBACK APP FOR PAMPANGA STATE AGRICULTURAL UNIVERSITY". Adviser: Maribel T. Bravo, MECPE This study, entitled Web-based customer feedback app for Pampanga State Agricultural University," is an online tool that enables universities to gather valuable insights from their customers about their products or services. It allows customers to share their opinions, feed back, and suggestions on various aspects of the business, including customer service, product quality, pricing, and more. The feedback received through the form helps the university identify improvement areas and make data-driven decisions to enhance its customer experience. With the ability to customize questions and analyze responses in real-time, web-based customer feedback forms have become a crucial tool for businesses to improve customer satisfaction and loyalty. This study aims to create a web-based app that will automate the customer feedback process, be capable of adding/deleting items on the questionnaire. compute the results of the customer feedback form, and generate the needed reports. The study may also examine the effectiveness of different types of feedback forms, question formats, and feedback mechanisms, as well as the impact of response rates on the quality of feedback received, and reduce manual record-keeping and work, making it easier for Pampanga State Agricultural University. In this study, the researcher will be using a questionnaire. It is a set of orderly arranged questions carefully prepared to be answered by a group of people and designed to collect facts and information. They will be using a survey questionnaire for the respondents of PSAU students that is based on the topic Web-based Customer Feedback App for Pampanga State Agricultural University. The students from different departments are the focus of this survey, the researchers must survey 64 students or more. They need at least 10 to 12 students from six different departments to determine the rating of the system. The study involved examining the feedback given by these respondents to learn more about their levels of satisfaction, preferences, and areas for development. Data gathering will take place for this project study. The need for developing the suggested system was determined after extensive investigation and data gathering. Based on the results gathered by the respondents, with a total mean of 3.9 from the admin, a total mean of 3.5 from the users, anda total mean of 4 from the IT experts, the researchers conclude that the study achieved its goals, and met the specific objectives of the system. Overall, developing an online feedback form system can help schools efficiently, accurately, and conveniently collect valuable feedback while protecting the privacy of respondents and providing data for analysis and decision-making.
