"DESIGN AND DEVELOPMENT OF WEBBASED COMMUNICABLE DISEASE MONITORING SYSTEM FOR RURAL HEALTH UNIT OF MAGALANG" Adviser: Ms. JACKILYN B. BACCAY The study entitled "Design and Development of Web-based Communicable Disease Monitoring System for Rural Health Unit of Magalang' aimed to facilitate the collection and submission of disease cases. The objective of the study are: to provide an online storage of disease data; to provide an interface that would allow the user to monitor which barangay has the most number of cases; to develop a system that would allow the user to track the reports points of origin; provide an interface that enable the user to search, view, and generate report; and to evaluate the system by end-users. The researchers used different techniques such as interview, Internet search and Survey Questionnaires to gather all the information needed in the completion of the study. The researchers adapted the Rapid Application Development methodology as it allows a fast development through the accelerated time cycle. For the design and development of the system, the researchers used different software such as Laravel as framework of PHP, HTML, Javascript and CSS. To determine the design and flow of the system, different diagrams were also used such as Context Diagram, Use Case Diagram, Data Flow Diagram, and System Flowchart. Through testing the approval of capabilities and functions of the system to the users, the researchers collected a good result in the survey conducted and the study gathered a grand mean of 4.0 which implies that the respondents strongly agreed that the developed system has successfully meet the set objectives.
